We provide enquiries about disability and dementia care, including daily living support and integrated medical and care services. Tell us which daily activities are hardest to manage, what support is already in place and what matters most to your family.

Describe daily needs clearly

Note any help needed with eating, dressing, using the toilet, moving around and everyday communication. Explain when changes began and the care already arranged. Describe what you observe; you do not need to diagnose or assign a care level yourself.

Organise existing examination, diagnosis, medication and allergy records by date. If documents are needed, confirm their purpose and the official receiving channel with our staff. Do not submit medical records, identity documents or imaging through public web pages.

Confirming services and fees

Ask about each service, admission conditions, responsible staff, daily contact, medical coordination and itemised fees. Beds, staffing, specific services and costs are subject to the arrangements and written explanation provided after discussion and assessment.

Daily living care and medical treatment are explained separately. For medical services, verify the actual medical institution and professionals providing them. This page does not provide individual diagnoses or treatment arrangements.
